Does exercise raise or lower blood sugar?

Muscle contraction moves GLUT4 transporters to the cell surface through a pathway that does not require insulin. That is why walking works even when insulin resistance is high, and why it is the most dependable non-pharmaceutical lever available.

Moderate activity — a brisk walk, easy cycling, gardening — typically produces a measurable drop within 30 to 60 minutes.

The effect also outlasts the session. Insulin sensitivity stays improved for up to 24 to 48 hours afterwards, which is why consistency matters more than any single workout.

When it goes up instead

High-intensity or competitive exercise — sprint intervals, heavy lifting, a race — triggers adrenaline and cortisol, which tell the liver to release stored glucose. That release can outpace what the muscles are using, so glucose rises during and shortly after.

This is normal physiology, not a failure. It usually falls afterwards, sometimes below where it started. If you consistently see a rise after hard sessions, that is the explanation.

Early-morning exercise can also read high because it coincides with the dawn phenomenon.

When not to exercise at all

Above 240 mg/dL (13.3 mmol/L) with moderate or large ketones. Without enough insulin, working muscle cannot use glucose, so the body burns fat instead and produces more ketones. Exercise makes ketoacidosis worse rather than better. Check ketones first if you use insulin and are running high.

Also skip it if you feel unwell, or have symptoms suggesting a hypo.

If you take insulin or a sulfonylurea

Exercise can lower glucose for hours afterwards, including overnight. Delayed post-exercise hypoglycemia is common and catches people out.

  • Test before, and after, while you learn your own pattern
  • Carry fast-acting carbohydrate
  • Discuss dose adjustments with your prescriber before starting a new routine — this is exactly the situation where a plan beats improvisation

What to aim for

The general target is 150 minutes a week of moderate activity, spread across at least three days, plus resistance training twice a week. Muscle is the body's largest glucose sink, so building it raises your capacity to store glucose rather than just spending it.

If that sounds like a lot, start with the highest-yield version: a 10-minute walk after your largest meal.
