What blood sugar level causes symptoms?
The threshold that explains most symptoms
Your kidneys filter glucose and reabsorb nearly all of it. That reabsorption has a ceiling — the renal threshold — at roughly 180–200 mg/dL (10–11 mmol/L) in most people. Above it, glucose spills into urine and pulls water with it.
That single fact explains the classic cluster: frequent urination, thirst, dry mouth, dehydration, and a good share of the fatigue and headache. Below that line, none of those mechanisms is operating, which is why so much high blood sugar is silent.
Why the threshold is not the same for everyone
Adaptation. People who have run high for months or years often feel unremarkable at 250 mg/dL. The same reading in someone whose usual number is 90 can be genuinely unpleasant. The body adjusts, which is a problem rather than a mercy — it removes the warning.
The renal threshold itself varies. It is lower in pregnancy, which is why glucose appears in urine at lower blood levels. It rises in some kidney disease. SGLT2 inhibitor medications deliberately lower it, which is the whole point of how they work.
Rate of change matters. A rapid rise produces more symptoms than the same number reached slowly.
The symptoms that appear at lower levels
Not everything waits for 180. Some effects show up in the prediabetes range or below:
- Fatigue after meals, from post-meal spikes even with a normal fasting number
- Recurrent thrush or urinary infections
- Acanthosis nigricans — velvety dark patches in skin folds, a visible marker of insulin resistance
- Slow-healing cuts
And damage does not wait for symptoms at all. Vascular and nerve changes begin at levels well below the symptomatic threshold, which is the core reason screening exists.
The practical conclusion
Do not use how you feel as a gauge. It is unreliable in both directions — you can feel fine at a genuinely harmful level, and feel terrible from something unrelated.
Test rather than estimate. If you are trying to work out whether a symptom is glucose-related, take a reading at the time you feel it and note both. A few of those pairs will tell you more than any threshold in an article.
