Is a fasting blood sugar of 105 prediabetes?
Where 105 sits
| Fasting glucose | Category |
|---|---|
| 70–99 mg/dL (3.9–5.5 mmol/L) | Normal |
| 100–125 mg/dL (5.6–6.9 mmol/L) | Prediabetes |
| 126 mg/dL (7.0 mmol/L) or above | Diabetes |
105 is near the bottom of the prediabetes band. Worth being honest about what that means: 105 is not biologically different from 98. These are lines drawn across a continuum for practical purposes, not cliff edges.
What makes it worth attention is the trajectory. Impaired fasting glucose is one of the strongest predictors of developing type 2 diabetes, and without intervention a meaningful proportion of people with prediabetes progress within a decade.
Confirm before concluding
One reading is not a diagnosis, particularly from a home meter, which is permitted a margin of roughly ±15% against a laboratory value. A true 100 can legitimately read anywhere from 85 to 115.
Ask for a lab fasting glucose and an A1c together. The two tests answer slightly different questions and disagreement between them is itself informative.
Make sure the fast was genuine, too — eight hours with nothing but water. Coffee with milk, a mint, or a late-night snack will distort it.
The good news, with a real number attached
Prediabetes responds to intervention better than almost anything else in this field. In the Diabetes Prevention Program, losing 5 to 7% of body weight combined with 150 minutes of weekly activity reduced progression to type 2 diabetes by 58% — better than metformin in the same trial.
For someone at 200 lb, 5 to 7% is 10 to 14 lb. That is a specific, reachable target rather than an aspiration.
The changes with the best evidence are unspectacular: walk after meals, eat vegetables and protein before starch, add fibre, protect your sleep, and treat any untreated sleep apnea. Our guide to lowering blood sugar covers what actually holds up.
What to ask for
Get re-tested annually at minimum. Ask whether you qualify for a structured diabetes prevention program — they exist, they are often free or low cost, and they outperform being told to eat better and come back next year.
